1. Acceptance of Terms
By accessing or using Arceum ("the Service"), you agree to be bound by these Terms of Service. If you do not agree, do not use the Service. "Arceum" refers to the website located at tryarceum.com and all associated tools, data, and content operated by Arceum LLC.
2. Description of Service
Arceum provides a denial code reference tool and AI-powered denial resolution platform for healthcare billing professionals. The Service publishes Claim Adjustment Reason Codes (CARCs) and Remittance Advice Remark Codes (RARCs) sourced from X12 and the Centers for Medicare & Medicaid Services (CMS). Arceum also provides interpretive guidance, resolution steps, appeal letter generation, and payer-specific intelligence.
3. Customer Warranty — De-identified Data
By using the Denial Resolver, appeal letter generator, or any tool that accepts claim data, you warrant that all data you upload or enter has been de-identified in accordance with HIPAA Safe Harbor or Expert Determination methods. Specifically, you agree not to submit:
- Patient names, addresses, or contact information
- Social Security numbers or Medicare Beneficiary Identifiers
- Dates of birth or other demographic identifiers
- Any of the 18 HIPAA identifiers
Arceum is designed to work with de-identified claim data only — denial codes, payer names, procedure codes, and billed amounts. You are solely responsible for ensuring that any data you submit complies with applicable privacy laws.
4. Data Ownership
You retain full ownership of all claim data you upload or enter into the Service. Arceum does not claim any ownership rights over your data. We use your data solely to provide the Service to you and will not use it for any other purpose without your explicit consent.
5. Data Handling and Processing
When you use Arceum's analysis tools:
- Input processing: Claim data you submit is processed by our rules engine to generate resolution plans. Uploaded CSV files are processed in memory and are not persisted after analysis.
- AI processing: When you request an appeal letter, de-identified claim data (denial codes, amounts, payer names) is sent to Anthropic's Claude API. This data is automatically deleted within 30 days and is never used for model training.
- Storage: Resolution plans and generated appeal letters are stored in your account on our database (Supabase) and are accessible only to you.
6. AI Disclosure
Arceum uses artificial intelligence (Anthropic's Claude API) to assist with denial analysis and appeal letter generation. AI-generated content is provided as a starting point and should be reviewed by a qualified billing professional before submission to payers. Arceum does not guarantee the accuracy, completeness, or effectiveness of AI-generated appeal letters. Human review and judgment are required for all billing decisions.
7. Sub-Processor Disclosure
The following third-party services process data on Arceum's behalf as part of delivering the Service:
- Anthropic (Claude API) — AI-powered appeal letter generation and denial analysis
- Supabase — application database and authentication
- Vercel — hosting and serverless infrastructure
- Loops.so — email delivery and contact management
- Resend — transactional email delivery
- PostHog — anonymous product analytics
8. Free Tool — Provided "As Is"
The denial code lookup tool is provided free of charge. The Service is provided "as is" and "as available" without warranty of any kind, express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, or non-infringement.
Arceum makes no guarantee that the Service will be uninterrupted, error-free, or that defects will be corrected. You use the Service at your own risk.
9. Data Sources — X12 and CMS
CARC and RARC code descriptions published on Arceum are sourced verbatim from the official X12 code lists and CMS quarterly publications. Arceum is not affiliated with X12, the ASC X12 organization, or the Centers for Medicare & Medicaid Services. Official sources:

Code lists are updated quarterly by X12/CMS. While Arceum endeavors to maintain current data, there may be a delay between official updates and their appearance on the Service. Always verify critical billing decisions against the primary official sources.
10. Not Professional Billing Advice
The content on Arceum — including code descriptions, resolution steps, appeal templates, payer notes, and all other materials — is provided for informational and reference purposes only. It does not constitute professional billing advice, legal advice, or compliance guidance.
You are solely responsible for your billing decisions, claim submissions, and compliance with applicable federal and state laws, payer contracts, and CMS guidelines. Consult a qualified billing professional or healthcare attorney for advice specific to your situation.
11. Limitation of Liability
To the maximum extent permitted by applicable law, Arceum LLC and its officers, employees, agents, and licensors shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including but not limited to lost revenue, lost profits, claim denials, payer audits, compliance penalties, or loss of data arising out of or related to your use of the Service.
Arceum's total liability for any claim arising from use of the Service shall not exceed the amount you paid Arceum in the twelve months preceding the claim, or $100, whichever is greater.
12. Indemnification
You agree to indemnify and hold harmless Arceum LLC from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys' fees) arising from your breach of these Terms, including but not limited to submitting data that contains PHI or other protected information in violation of Section 3.
Arceum agrees to indemnify you from any claims arising from Arceum's breach of its data handling obligations as described in these Terms and our Privacy Policy.
13. Breach Notification
In the event of a data breach affecting your information, Arceum will notify affected practices within 72 hours of confirming the breach. Notification will include the nature of the breach, the categories of data affected, and the measures taken to address and mitigate the breach.
14. Data Deletion
You may request full deletion of your account and all associated data at any time by emailing privacy@tryarceum.com. Upon receipt of a confirmed deletion request, Arceum will permanently delete your account, resolution plans, appeal letters, and any other stored data within 30 days. Deletion from third-party sub-processors (email platforms, analytics) will be completed in accordance with their respective retention policies.
15. Acceptable Use
You agree not to:
- Scrape, crawl, or systematically download Service content at scale
- Use the Service to train AI or machine learning models without written permission
- Attempt to reverse-engineer or disrupt the Service infrastructure
- Submit false or misleading information through any form on the Service
16. Intellectual Property
The Arceum brand, logo, original content (resolution guidance, payer notes, blog posts, appeal templates), and software are the intellectual property of Arceum LLC. Official CARC/RARC code descriptions are published by X12 and CMS and remain their respective property. Nothing in these Terms grants you a license to reproduce or distribute Arceum's proprietary content.
17. Governing Law
These Terms are governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law provisions. Any dispute arising from or related to these Terms shall be resolved exclusively in the state or federal courts located in Delaware.
18. Changes to These Terms
We reserve the right to modify these Terms at any time. Material changes will be communicated via email to registered users. Continued use of the Service after changes are posted constitutes acceptance of the revised Terms.
20. Payer Resolution Intelligence
20.1. Information sources. Arceum provides denial resolution guidance, appeal process information, contact details, and workflow recommendations specific to healthcare payers including but not limited to UnitedHealthcare, Aetna, and Blue Cross Blue Shield of Arizona. This information is derived from publicly available provider administrative guides, operating manuals, and educational materials published by these payers for use by healthcare providers.
20.2. No payer affiliation. Arceum is not affiliated with, endorsed by, sponsored by, or partnered with any healthcare payer. Payer names, plan names, and trademarks referenced on this website are the property of their respective owners and are used solely for the purpose of identifying the payer to which specific resolution guidance applies.
20.3. Informational purpose only. All payer-specific resolution guidance, phone numbers, portal URLs, mailing addresses, appeal deadlines, and process descriptions are provided for informational and educational purposes only. This information does not constitute legal advice, financial advice, or a guarantee of claim payment or appeal outcome. Payer policies, processes, contact information, and deadlines are subject to change without notice by the payer.
20.4. No guarantee of outcomes. Following the resolution steps or other recommendations provided by Arceum does not guarantee that a denied claim will be overturned, paid, or resolved in the Customer's favor. Claim outcomes are determined solely by the applicable payer based on the member's benefit plan, applicable laws, and the payer's own policies.
20.5. Customer responsibility. Customer is responsible for: (a) verifying that payer-specific information displayed by Arceum is current before acting on it; (b) consulting the applicable payer's current provider manual or portal for the most up-to-date requirements; and (c) complying with all applicable payer contracts, state laws, and federal regulations governing claims submission, reconsideration, and appeal.
20.6. AI-generated content. Certain content on this website, including but not limited to appeal letter drafts, resolution step summaries, and denial code explanations, is generated or assisted by artificial intelligence. AI-generated content is reviewed for accuracy but may contain errors. Customer should review all AI-generated content before submission to any payer and should not submit AI-generated appeal letters without reviewing them for accuracy and completeness. Arceum is not responsible for errors in AI-generated content or for adverse outcomes resulting from the use of such content.
20.7. Data currency. Payer resolution intelligence displayed on this website is derived from payer administrative guides current as of the date indicated on the relevant content. Arceum endeavors to review and update payer-specific information at least quarterly but does not guarantee that all information reflects the payer's most current policies.
20.8. Limitation of liability for payer information. Arceum shall not be liable for any loss, damage, claim denial, missed filing deadline, or adverse outcome arising from Customer's reliance on payer-specific information provided by Arceum, including but not limited to: incorrect or outdated phone numbers, portal URLs, or mailing addresses; expired or changed appeal deadlines; discontinued submission methods; or changes to payer policies not yet reflected in Arceum's content.
